​Please note - before the notary comes to the signers location:

• The signer must have a valid unexpired government issued ID (ex: drivers license, state ID, passport, etc.)

• The signer must be able to understand the content and purpose of the document

• The signer must sign the document voluntarily and be able to write their name without any help (a thumbprint may be used if needed)

• The signer must not be affected by psychoactive substances such as painkillers, drugs, tranquilizers, alcohol etc.

• If witnesses are needed -

  • Please ensure that the witness is present

  • A witness must be 18 years or older, of sound mind, and have no interest in the parties ​

  • May not be related to the signer by blood, marriage, or adoption

  • May not be entitled to any part of the signers estate, by will or operation of law, at the time the document is signed

  • May not be appointed as the signers agent

  • May not be involved with provision of the signers health care at the time the document is signed

• The notary may request family leave the room, leaving just the witness(es) present
